The health of our insured persons is important to us. That is why we support them on their individual journey – whether that means staying healthy, recovering from an illness or learning to live with a medical condition.

Health support

We contribute towards activities that keep them fit and healthy, offer guidance in making informed healthcare decisions and provide orientation. This also includes information about generics and biosimilars, for example.

In your role as a service provider, our care management services and health programmes may also be of interest – to support and complement the treatment you provide.

Care management

As a doctor or member of a hospital social services team, you discuss with your patients the plan for their discharge and organise follow-up treatment. If this follow-up treatment is extensive or the overall situation is complex, it can be helpful to offer support both before and after discharge to ensure successful treatment outcomes. After all, for many patients it’s a huge challenge to navigate the healthcare system on their own.

In such cases, CSS offers insured persons its care management services. A specialist will work with you and the patient to clarify questions such as:

  • Is it appropriate/possible for the patient to return home, and what follow-up solutions are available there?
  • Who will support the patient if changes are needed to Spitex services and/or outpatient therapies?
  • What medical aids can we use, and what financing is available?
  • Who will offer the relatives, who may be out of their depth, a helping hand?

Successful partnerships

In most cases, we contact our insured persons directly when they require support. However, we are also looking to collaborate with doctors and hospitals to identify those in need of support at an early stage.

CSS already has successful care management partnerships with organisations that include the Swiss Paraplegic Centre in Nottwil, REHAB Basel (a clinic for neurorehabilitation and paraplegiology), Cliena Littenheid and Clinica Hildebrand.

Health-related offers

As a health partner, we support our insured persons even in difficult healthcare situations. We offer services that can have a positive impact on health and prevent illness – by encouraging personal responsibility and promoting health literacy. The aim is to support prevention, recovery and living with a health condition. The services are designed to complement, not replace, basic care.

Partnerships

To offer our insured persons innovative services, we work with a range of providers and partners. A partnership is established when the service is market-ready and its effectiveness can be scientifically proven. It is also important that the service is easily accessible for the intended target group. We do not support services which are already covered by basic insurance, or which are free of charge for other insured persons.

Health notebook: all about your child’s health

The health notebook, developed in collaboration with Swiss paediatricians, contains information on all aspects of children’s health. It includes valuable tips on nutrition, allergies and accident prevention, along with ample space for important notes – from a child’s first steps through to medical check-ups.

CSS has been supporting pädiatrie schweiz, the publisher of the notebook, in its production and distribution for many years.

The notebook is published once a year and is distributed in hospitals, clinics, paediatric practices and by midwives.
